Nuss procedure
Purpose: Two metal alloys (stainless and titanium) are commonly used as a pectus bar in Japan. However, contact-allergy to various metals has been reported recently. Preoperative evaluation of contact-allergy to metals seems to be helpful for the choice of metal alloys for the treatment of pectus excavatum (funnel chest) using Nuss procedure. Patients and Methods: We had screened 48 patients of pectus excavatum using a patch test from Jan. 2001 to Dec. 2005. Among the 48 patients, 35 were male and 13 were female. The ages of the patients ranged from 5 to 16 years with the mean age of 8.6-years at the time of operation. Results: Ten out of the 48 patients (21%) showed positive reactions to stainless steel alloy elements. However, the positive reaction to titanium alloy was detected only in one out of the 35 patients (3 %) examined. This showed a statistical significance (p<0.015). In 4 cases, multiple positive reactions were observed. Nickel was the most common allergen metal (19%) in all the allergens, followed by manganese (6%), and chrome (4%). In 9 out of 10 cases, titanium alloy was used for the Nuss procedure, and their postoperative courses were uneventful. In one case out of 10 showed positive reaction to aluminum that is an element of titanium alloy. This case also revealed positive reactions to nickel, chrome, manganese, copper and iron. Conclusion: The preoperative screening patch test is useful for the detection of metal allergy and helpful for choosing the type of metal alloys for the treatment of patients with pectus excavatum.
