Measurement of regional cerebral blood flow (CBF) was done, using N-isopropyトp-[^<123>I]-iodoamphetamine(IMP)on 24 patients with dementia of Alzheimer type (DAT)and 13 age-matched controls. We used SPECT and arterial blood sampling for quantitative measurement of regional CBF. The same psychiatrist estimated clmical state and dementia ratig. We divided DAT patients into two groups according to the Clinical Dementia Rating(CDR). One was mild and moderate DAT group(12 patients, mean age 81.3), and the other was severe DAT group(12patients, mean age 82.6). Mean CBF of cerebral cortex was 47.3ml/100g/min in the normal control group, 46.0ml/100g/min in the mild and moderate DAT group and 37.2ml/100g/min in the severe DAT group. There was significant difference between normal control group and severe DAT group(p<0.01). No significant difference was seen between mild and moderate DAT group and normal control group. Relative CBFs were calculated as ratios of regional activity to cerebellar activity on SPECT image. Ratios of mean cortical activity to cerebellar activity revealed significant difference between normal control group and severe DAT group(p<0.01). There is no significant difference between mild and moderate DAT group and normal control group. On quantitative measurement method, CBF were globally reduced in all the regions of brain except brain stem in severe DAT group. Relative CBF study revealed CBF reduction in frontal, parietal, temporal and occipital cortex but no CBF reduction in cerebellum, precentral gyrus and deep gray matter in severe DAT group. Relative CBF study underestimated CBF reduction when compared with the quantitative method. The cause for this was reduction of mean CBF of cerebellum in severe DAT group compared with normal control group.
