[原著]向精神薬療法中の精神神経疾患患者における拒薬に関する研究

Medication should be done in accordance with instructions for rationally treating patients with psychotropic drugs. In this study, hospitalized patients who resisted medication (drug noncompliance) were examined. Patients had chronic schizophrenia and had undergone psychotropic drug treatment at 6 different psychiatric hospitals. The noncompliant patients were 173 (11.6%) out of a total of 1491 patients. They were categorized as follows: medication with advice, 97 (56.1%); medication with instruction, 28 (16.2%); medication with repeated instruction, 17 (9.8%); complete resistance against medication, 17 (9.8%) and forced medication, 14 (8.1%). The noncompliant patients were compared with two groups of drug compliant patients: patients who accepted、 medication voluntarily and those who involuntarily accepted medication. Those who accepted medication involuntarily were considered the control group. Significant differences were seen in the mental state regarding hallucinations and delusions, and also with regard to negative symptoms. In the Brief Psychiatric Rating Scale (BPRS) for the noncompliant group, the total score as well as in the scores for hallucination, anxiety, excitement, tension, and hostility, were higher than that of the controls. Drug noncompliance influenced by the patients themselves was considered to be due to lack of insight, 48 out of 173 (27.8%); pathological experiences, 32 (18.5%); anxiety about drugs, 29 (16.8%); and side-effect of drugs, 21 (12.1%). Drug noncompliance was also influenced by the family members in some scores; 8 (4.6%) reported the family advised noncompilance, 7 (4.0%) reaction against the familys instructions about medication, and 5 (2.9%) reaction against forced hospitalization. Drug noncompliance also resulted from the actions or attitudes of hospital staff; 11 (6.4%) showed reaction against forced hospitalization, and 4 (2.3%) showed unfavorable relationships with the comedical staff in the institutions. These findings show the importance of understanding the problem of drug noncompliance.
