We experienced seven operated cases in atlanto-axial dislocation during past five years, and reported three interesting cases in this paper. Case 1 : A 12 year-old girl with neurofibromatosis had tetraplegia caused by atlantoaxial dislocation. Good result was obtained by modified McGraw's method. But at two years after operation, patient showed progressive paraparesis. The cause of this paraparesis was spinal cord tumor at T_<4.5.6> which was demonstrated by tomograms and CT scanning. Case 2 : A 23 year-old man had tetraparesis due to atlanto-axial dislocation with hypoplasia of the dens. A myelogram showed a complete block at the level of atlanto-axial joint, and lumbar spinal canal stenosis, partial block at the lumbosacral region. In addition to the upper cervical spinal cord injury, the occurence of this lumbosacral lesion may be related to the pathogenesis of the leg weakness and foot deformity Case 3 : A 38 year-old femal had atlanto-axial dislocation with fracture of the dens. This is a rare case as atlanto-axial dislocation due to rheumatoid arthritis. Generally speaking rheumatoid infammation are liable to affect the synovial joint of the atlanto-axial joint and causes subluxation of the foint without fracture of the dens.
