A case of leiomyoblastoma of the pylorus showing prolapse into the duodenal bulb and cholelithiasis in a 56-year-old female is described herein. The patient was hospitalized to the University Hspital for further investigation with a diagnosis of a polypoid tumor of the pylorus on April 18, 1983. Upper gastrointestinal series demonstrated a polypoid tumor in the pylorus to the duodenal bulb. Her gastrofiberscopy revealed a submucosal tumor overlying the pylorus and obstructing the outlet of the stomach. She was also diagnosed to have cholelithiasis. At surgery, the polypoid tumor showed no evidence of tumor spreading onto the serosa and to the lymph nodes. Following cholecystectomy for cholelithiasis, antral gastrectomy was carried out. The polypoid tumor was located over the pyloric ring and pedunculated, with projection into the duodenal bulb. It had a smooth mucosal surface, and was oval in shape and 5×3×2cm. in size. On cut section, the tumor appeared lobulated, locally necrotic and circumscribed between the mucosa and the muscularis propriae. The pathological report was leiomyoblastoma. At this writing, she has been doing well about three years since surgery. This is the second reported case showing prolapse into the duodenum and the first of the pylorus in Japan.
