We analyzed 81 divers to study dysbaric osteonecrosis of the femoral head (DOF). One hundred and sixty hips were evaluated by roentgenogram. Fifty-eight hips (36.3%) showed bone lesions. We divided these bone lesions into several groups according to Kawashima's classification, as follows: one hip with type A1 (segmental opacity), ten with type A2 (linear opacity), five with type A3 (mass opacity), one with type A4 (sequestration of the cortex), two with type A5 (collapse of the cortex), two with type A6 (osteoarthritis), ten with type B1 (localized calcification), nine with type B2 (irregular calcified areas), 15 with type B3 (transradiant areas and cystic lesions) and three with type C (dense areas like an egg shape). Following this, 85 hips out of the 160 hips were evaluated by MRI. Forty-one hips (48.2%) showed low signal intensities on T1-weighted MRI. Out of the 41 hips, 4 hips were judged as normal and 37 hips revealed abnormalities on roentgenogram. There was no difference in the incidence of positive findings between MRI and roentgenogram, however in 25 hips (29.4%) MRI showed a distinct low signal intensity in a site that was not detected on the roentgenogram, yet we were not able to identify this lesion as false positive or not in this study. Analysis of the questionnaires from all divers to survey related factors affecting DOF revealed that the experience of bends and fukashi (repetitive dive for improving the symptoms of decompression sickness), hip pain, obesity and drinking were risk factors of DOF.
