We present two cases of orthodontic treatment in our department and show a corrective solution to various orthodontic problems. Case 1 was a left side C.L.P. treated with cheiloplasty at age 4 months, palatoplasty at age 1 year and 11 months and secondary bone grafting (S.B.G.) at age 11 years and 2 months. After S.B.G., we started orthodontic treatment with an upper dental arch expansion appliance at age 13 years and 5 months. The patients was treated with cheiloplasty at age 13 years and 7 months and at age 16 years and 11 months. The multi-bracket treatment technique started at age 14 years and 1 month. This was changed, to a retainer at age 17 years and 4 months. The retention period was 10 months, and the orthodontic treatment period was 3 years and 11 months. Case 2 was a bilateral C.L.P. treated with cheiloplasty at age 5 months and 9 months. This was followed by and palatoplasty at age 1 year and 8 months. Orthodontic treatment started with upper dental arch expansion at age 14 years and 2 months and we began the multi-bracket treatment technique and used a retainer at age 19 years and 7 months. Meanwhile, the patient was treated with cheiloplasty by the Abbe method at age 16 years and 11 months. This was repeated at age 17 years and 6 months. At when 20 years and 11 months, we used the implant technique at the same time we performed S.B.G. and fistura closing. Because orthodontic treatment began S.B.G. comparatively late, we performed cheiloplasty twice over the orthodontic treatment period. There was an interruption due to the operation and orthodontic treatment became comparatively late at age 5 years and 5 months. The retention period was 3 years and 10 months. It is necessary for occlusal management to continue observation carefully in future. With the demand for improvement in teeth alignment from speech therapists and for upper dental arch expansion before S.B.G. from surgeons, there are many occasions when we start orthodontic treatment early. Thus, we conduct comprehensive orthodontic treatment based on the patient-s existing conditions.
