A 21-year-old woman was admitted to the hospital because of a chest pain and shortness of breath. A chest X-ray film revealed a spontaneous pneumothorax with complete collapse of the left lung. The collapse of the lung was caused by the rupture of bullae in the upper lobe. A closed catheter drainage with continuous suction was performed. However, acute re-expansion pulmonary edema with a cough and copious foamy sputum occurred immediately after the start of the catheter drainage. Thepatient fell into hypovolemic shock because of copious foamy sputum. The pulmonary edema was successfully treated with fluid transfusion wihtout use of corticosteroids and diuretics. Bullectomy was performed three weeks after the catheter drainage, since the pneumothorax recurred.
