The patient had a history of 12 years' duration of excessive sweating of the hands. In surgery, he was placed in semi-Fowler's position under general anesthesia with endotracheal double lumen tube intubation. After deflation of the right lung, the 5mm trocar for introduction of the thoracoscope was introduced into the 4th intercostal space on the midaxillary line. Through the thoracoscope, the sympathetic trunk on the ribs and ganglions at the lower borders of the corresponding ribs were found easily. The other two trocars were introduced into the 5th and 6th intercostal space around the axillary line. The pleura covering the segments of the sympathetic trunk was incised longitudinally, and the T2-T5 sympathetic ganglions were dissected, stripped with grasping forceps and coagulated with diathermy. Following the right sympathectomy, the left sympathectomy was done using the same procedure without coagulation.The chest tubes were placed. The patient was satisfied with the control of hyperhidrosis palmaris achieved, and was discharged on the 3rd postoperative day.
