A 60-year-old man developed upper abdominal pain on April 4, 1992, when upper gastrointestinal endoscopy revealed a tumor of the papilla of Vater. Microscopic exanimation of biopsy specimens from the tumor showed adenocarcinoma. He was admitted to the Ryukyu University Hospital on June 14 for surgery. Sigmoid carcinoma with a hepatic metastasis was incidentally detected at laparotomy. Thus, pancreatoduodenectomy and sigmoidectomy were performed. Pathologic examination showed papillary adenocarcinoma in the papilla of the Vater and moderately differentiated adenocarcinoma and tubular adenoma in the colon. He has been doing well for the past 2 years since surgery. There was no family history of familial adenomatosis coli. It is generally accepted that adenomas of the papilla of Vater and duodenum occur highly in patients with familial adenomatosis coli.
