One of the major differences between the two systems is the structure of the applicator. Flexible applicators are used in the ^<60>Co system but rigid ones in the ^<192>Ir system. Although there are many reports on clinical and physical aspects of HDR-RALS, to our knowledge there is no report on the effect of the structure of the applicator. Our purpose is to study the effects of the structure of the applicator in relation to its arrangement in the treatment of carcinoma of the uterine cervix. Thirty-three patients with uterine cervical cancer treated with HDR-RALS were studied. Twenty-one cases were treated with the ^<60>Co system and 12 cases with the ^<192>Ir system. Comparisons between ^<60>Co and ^<192>Ir systems were made for 1) the slant angle of the tandem in anteroposterior and lateral planes, 2) the distance of left and right ovoids, 3) the ratio of left and right tandem-ovoid distance. The slant angle was -4° for ^<60>Co and 23° for ^<192>Ir in the anteroposterio plane, and 8 for ^<60>Co and 3° for ^<192>Ir in the lateral plane. The separation of the ovoid was 2.7 cm for ^<60>Co and 2.5 cm for ^<192>Ir. The ratio of the left tandem-ovoid distance to the right tandem-ovoid distance was 1.16 for ^<60>Co and 0.99 for ^<192>Ir.
