During the operation, massive bleeding from the prostate gland occurred, and was controlled by the replacement of transurethral balloon catheter. Recurrent bleeding episodes necessitated imaging study of the bladder. CT scan and MRI showed abnormal vessels in the pelvic space, and angiography revealed nidus feeding from bilateral internal iliac arteries, inferior mesenteric artery, and draining to dilated internal veins. Saccular aneurysm of inferior mesenteric artery was also seen. Transcatheter arterial embolization (TAE) was performed on the arteriovenous malformation by N-butyl-cyanoacrylate (NBCA) and on the saccular aneurysm by steel coils.
