Mothers attending infant physical examinations were surveyed about family plannning practice and attitudes. A total of 1,165 participants (839 from Naha City and 326 from Urasoe City; average age of 30) were involved between December 2000 and March 2001. Results were compared with a 1988 survey in the same 2cities in Okinawa and with the nationwide research by Mainichi Newspaper [25^th Report on Family Planning in 2000]. The ideal number of children were higher (2.76 in 1988, 2.95 in 2000) although the the present number of children has decrease (1.88 in 1988, 1.86 in 2000). The survey showed certain changes in the mothers' perceptions : There is a higher acceptances of artificial abortion (2.7% in 1988, 16.0% in 2000), and in vitro fertilization (65.1% in 1988, 81.3% in 2000). However the actual rate of artificial abortion was not significantly different between the two years. In cases of pregnacy dispite the use of contraceptives, the proportion of mothers who continued their pregnancy were significantly lower (82.8% in 1988, 77.9% in 2000). Okinawa still has the highest birthrate in Japan. However, this is decreasing for a number of reasons, including economic factors and the deterioration of the childcare and educational environment. Caregivers must assist the family throught such traditional support as the local yuimaru, where the people in the community share in child care, and through guided family planning to enhance the quality of couples' lives and to improve their well-being throughout the childbearing years.
