The frequency of nocturia increases with aging in both males and females. Its etiology is divided into nocturnal polyuria and a decrease of nocturnal bladder capacity. From our previous studies, the causes of nocturnal polyuria include excessive intake of water and/or hypertension. For a decrease of nocturnal bladder capacity, hypertension, sleep disturbance, and/or functional disturbance of the micturition inhibitory mechanism in the brain and spinal cord can be considered as causes. Both nocturnal polyuria and a decrease of nocturnal bladder capacity are involved in the occurrence of nocturia in many elderly persons. A bladder diary is useful for the diagnosis of nocturia and its cause. As treatment of nocturnal polyuria, restriction of water intake, administration of antihypertensive drugs or diuretics in the daytime, administration of vasopressin before sleep, and walking in the evening are performed. As treatment for a decrease of nocturnal bladder capacity, bladder training can be used as well as administration of anticholinergics, naftopidil, flavoxate, or hypnotics.
