 note = {For urgent need of blood access in case of congestive heart failure or shunt trouble in chronic uremia, immediate construction of an internal. A-V fistula is desired. However, when it was found impossible due to the poor state of vessels or general condition, one has to find a leeway for overcomming the situation. A new method of primarily making combined Quinton-Shaldon type cannulations immediately followed by an internal A-V fistula in starting or continuing hemodialysis has been quite satisfactory in such difficult cases in out hand.
