The case was a 44-year-old male affected with rheumatic endocarditis. On February 17, 1977, he began to suffer suddenly from left paresis without unconsciousness. He was admitted to the hospital on the suspicion of cerebral infarct. Angiography of the right common carotid artery revealed the occurrence of intracerebral hematoma and an aneurysm 3 × 5 mm in size in the periphery of the pre-Rolandic branch of the ascending frontopanetal artery originate from the A. cerebn media. CT scan disclosed the presence of an area of high density with a distin demarcation in the right frontoparietal region and an area of low density around that area. The present case was diagnosed as bacterial aneurysm and intracerebral hematoma produced by rupture of the aneurysm. Then an operation was performed to remove the aneurysm and hematoma. The patient took a satisfactory course after the operation, manifesting a marked lmprovement in clinical symptoms.
