The patient had had an episode of a juvenile type of diabetes mellitus and was treated with insulin for about 18 years. Her initial symptoms were pain and discharge of the right ear. The bacteriological study demonstrated Pseudomonas aeruginosa infection and the histological finding of the granulation tissue from the external auditory meatus was  a nonspecific inflammation . The patient was admitted on January 6, 1982, and was underwent a radical mastoidectomy. The mastoid and tympanic cavity was almost intact, and the granulation tissue of the external auditory meatus was to emerge from the defect of its floor. The patient was suspected of a malignant external otitis and treated with 1 ) a careful diabetic control, 2 ) systemic administration of antibiotics and 3 ) local debridement.
