This patient was a high aged woman ( 79 years old ) and thought to be one of the monozygotic twins. The most important finding of physical examination was continuous murmur audible characterized especially by superficiality and localization. Correct diagnosis was done by selective coronary angiography, but coronary arterio-venous fistula should be thought to exist when the abnormal dilation of main stem of coronary artery was shown by supravalvular aortography. This patient had an anomaly of coronary artery, but her sister (another of the monozygotic twins) had normal coronary artery. This result indicates that the cause of anomalous origin of coronary artery fistula from pulmonary trunk may not be genetic factor, but some uterine factor or factors as yet unknown.
