Their residents and the government had coped with nursing care for elderly under the undeveloped healthcare system. There is a traditional culture to wash the human bones after a few years of burial on Okinawa islands without a crematorium. Research showed the death, burial situations, residents view of traditional burial and washing the bones, and intentions for the term of nursing care needs on five islands near Okinawa mainland. There are high percentages among Zamami island residents to get cremated. Behavioral pattern of hospitalizing and cremating the elderly in their terminal stages was speculated. A study within 20 years shows that after 2007 the interment of the death on the island has been changed to cremation. However, more than 50% of young males from Aguni Island have washed human bones. They have strong regional support for funerals and traditional events. They continue the interment. Residents living on islands that had a nursing home and had family living outside of the island preferred to live with their family outside their island than on the island in the nursing home. On small islands, the main care givers and relatives of elderly had difficulties with nursing care due to poor services on the island. They also had difficulties visiting the hospital and providing care to the elderly when living outside the islands.
