We report a case of a 77 year-old-woman who was diagnosed with left lung atelectasis caused by pulmonary TB that has been resolved for 42 years after inadequate treatment. Chest radiography and computed tomography revealed the left lung\u02bcs atelectasis and a compensatory expansion of the right lung, making the left main bronchus narrowed with irregular bronchial wall and pleural calcification. The patient did not show any apparent symptoms associated with atelectasis and pulmonary TB, and the pulmonary functon of the patient revealed almost normal. Serum levels of ESR, TNF-\u03b1 and IFN-\u03b3 were not elevated, but M.tubercurosis-specific- IFN-\u03b3 release assays (IGRA) showed positive.
