We investigated the remote effects of a small deep ischemic lesion on the cortical circulation in 15 patients with lacunar infarctions. We included only patients in whom computed tomography (CT) confirmed the presence of single lacunar infarct in the unilateral putamen (right 3 cases, left 5 cases) or posterior limb of the internal capsule ( right 4 cases, left 3 cases ). Regional cerebral blood flow (rCBF) was measured using the two dimensional 133Xe inhalation technique during subacute or chronic phase of stroke. Fifteen patients were aged 45-74 years old, and they suffered from vertigo, headache, or mild hemiparesis. We also measured rCBF in 18 controls aged 43-81 years old who had normal CT apperances, no brain disorders, hypertension, or diabetes by history. All 15 patients and 18 controls were right-handed. Difference of mean CBFs between ipsilateral and contralateral hemispheres was not obtained significantly. However, the rCBFs decreased around the bilateral central sulci in the patients with a putaminal infarction, and decreased at the bilateral parietal lobes in the patients with a capsular infarction. The patients with a putaminal infarction also presented a focal hyperperfusion area at the ipsilateral posterior part of lateral sulcus. Small lesions as lacunar infarcts, apparently effect the distant cortical circulation. And these effects depend on the location of lacunar infarctions. Furthermore, these effects were observed not only cortical hypoperfusion but also cortical hyperperfusion. Our results suggest these phenomena result from the transneural depression or activation. |
