Corrected QT(QTc) intervals were examined in 120 diabetic patients (35 IDDM and 85 NIDDM), 28 patients with coronary artery disease(CAD) without diabetes and 58 normal subjects. There was a linear relationship between the extent of diabetic autonomic neuropathy (D-AN) reflected by the autonomic function score and the QTc intervals (r=0.69, p<0.001). Though there was no difference of QTc interval between IDDM and NIDDM, QTc interval was prolonged when D-AN was presented in both types of diabetes. Patients with both diabetic microangiopathy (retinopathy and nephropathy) and D-AN showed significant prolongation comparing to the diabetics without these complications. However, there was no relation between the QTc intervals and diabetic microangiopathy itself. QTc interval of CAD patients with D-AN during treadmill exercise test was statistically prolonged comparing to both CAD patients without diabetes and CAD patients without D-AN. Significant correlationship between the duration of diabetes and the QTc interval was observed, while it was not related with the parameter of glucose or lipids metabolism. These results suggest the direct influence of D-AN on the prolongation of QTc interval, though its pathophysiological mechanism remains unclear. |
