A case of advanced cholangiocarcinoma of a 63-year-old female surviving by multidisciplinary treatment is reported. The patient developed mild jaundice in 1988. She was reffered to the University Hospital for treatment. Diagnosis was an advanced cholangiocarcinoma involving segment 5 of the anterior segment of the liver with total bilirubin, carbohydrate antigen (CA) 19-9 levels of 6.0 mg/ml and 54.8 IU/L, respectively. The patient underwent right trisegmentectomy and was initially treated with adjuvant chemotherapy. Histologically, the tumor was moderately to poorly differentiated adenocarcinoma with vascular invasion of the liver. Four years and five months later, CT scan of the abdomen revealed a recu汀ence of the tumor in the remnant liver and the left lung. We performed surgery again followed by treatment with adjuvant chemotherapy. She achieved partial response to the treatment. Eighteen months after the second surgical operation, chest X-ray and CT scan revealed multiple metastatic tumors in the left chest. The patient is alive with recurrent disease and currently being treated with regular chemotherapy. |
