There are several complications reported in ACL reconstruction. Of them, tunnel enlargement has been reported in recent years. The aim of this study is to investigate the effects of alendronate on bone tunnel enlargement after tendon graft model in rabbits. Materials and Methods: A total of 51 skeletally-mature, male Japanese white rabbits were used. The rabbits were divided into three groups: a control group (N=24), an alendronate group (N=24) and time zero (N=3). Alendronate (0.8mg/kg) was administered systemically during the operation. A bone tunnel was made at the proximal tibia with a drill bit. A graft was passed through a bone tunnel and fixed. Each of twelve rabbits was killed at 3 or 6 weeks after surgery. Eight tibia-tendon complexes were harvested for examination with micro-CT; the other four specimens were used for bone histomorphometric evaluation. Three rabbits were killed immediately after surgery and used for time zero micro-CT analysis. Results: The tunnel cross-section area was 3.54 mm^2 at time zero. That became 5.03mm^2 at 3 weeks, 4.23mm^2 at 6 weeks in the control group and 3.65mm^2 at 3 weeks, 2.39mm^2 at 6 weeks in the alendronate group. The tunnel cross-section area was significantly smaller in the alendronate group than the control group. Histologically, the eroded surface was 37.0% at 3 weeks, and 52.1% at 6 weeks in the control group, and 18.7% at 3 weeks, and 47.3% at 6 weeks in the alendronate group. Osteoclast number was 2.6/mm^2 at 3 weeks, and 2.2/mm^2 at 6 weeks in the control group, and 0.8/mm^2 at 3 weeks, and 1.6/mm^2 at 6 weeks in the alendronate group. The number of minimodelings representing bone formation that occurred on quiescent bone surface without preceding bone resorption was 0/mm^2 at 3 weeks, and 0.7/mm^2 at 6 weeks in the control group, and 7.4/mm^2 at 3 weeks, and 7.3/mm^2 at 6 weeks in the alendronate group. There are fewer osteoclastic activities and more trabecular minimodeling in the alendronate group than the control group. Discussion: The present study demonstrated that alendronate prevented bone tunnel enlargement at 3 weeks. In addition, we observed more trabecular minimodeling in the alendronate group than the control group.
