We report a case of solitary mass lesion in which a brain tumor was suspected but was diagnosed as demyelinating disease. A 67-years-old woman who presented with aware of aphasia and weakness of the right upper limb. About two month later, her symptoms were worsened and had gait disturbance due to weakness of her right lower limb. Gadolinium enhanced-head MRI revealed a solitary mass lesion, which showed horse-shoe like enhancement suggestive of neurodegenerative disease. However, negative oligoclonal band and lower than 0.4 IgG index in cerebrospinal fluid, no dissemination of space and time findings on head MRI, are altogether not suggestive of demyelinating disease. Thus, to have a definite diagnosis, we decided open biopsy. Histological findings were compatible with demyelinating disease without neoplastic cells. After the operation she suffered from recurrent lesion in the corpus callosum and corona radiata, we diagnosed with multiple sclerosis. Based on the clinical course of dissemination of space and time, steroid pulse therapy was performed.
