| 内容記述 | 小児がん等で突然長期入院になり、かつ、治療や補装具等でADLが低下して病床での生活を送っている児童は、以前できたことができなくなったことや自由に活動することが困難になる。そのため、家族以外の他者との関わりを拒んだり、学習に対する意欲を持てなくなったりするなど、他者や学習、外界へ「向かう力」が低下し、長期入院に伴う心理的不安や苦痛を抱えて過ごすことが多い。このように、心身ともに不安定な状態の児童に対して病院内での教育的支援を行うのが病院内訪問学級である。そして、病院内訪問学級担任の役割のひとつとして、家族や医療スタッフとの連携を図って児童との関係性を築いて童のADLを高め、その後の他者や学習に対する「向かう力」を引き出し、児童の心理的安定に寄与することがある。そこで、児童の心理的安定を図るための手立てとして、瀬底らが提唱する発達障がい児へのトータル支援(TGS:トータルグループサポート)の理念を活用した。本研究では、1年間の長期入院中の小学校高学年児童のADLを高め、他者や外界への「向かう力」を高めた実践について報告する。 | |||||||||

| 内容記述 | Children admitted to hospital for long-term bed care，series of treatments and usage of supportive devices due to severe illness like childhood cancer，find it hard or impossible to continue to perform actions that they could do before，resulting in lower levels of ADL. As a result，they may become withdrawn，have reduced motivation to move forward，suffer from high levels of anxiety and psychological pain，refuse to interact with others outside the family，or lose interest in learning. The “in-hospital lessons" are designed to provide educational support to children staying at a hospital for long-term bed care and suffer from the uneasy situation physically and psychologically. One of the important roles for teachers assigned to visit these children is to build relationships with them，coordinating with family and medical staff to increase their ADL，which in turn encourages children to take more“voluntary initiatives". The report documents an example in which coaching and supporting based on“the philosophy of holistic support*" have increased the level of ADL，and motivated late elementary graders who had stayed at a hospital for more than a year to take more“voluntary initiatives". * Sesoko advocates “the philosophy of holistic support" for the psychological stability of children with developmental impairments. | |||||||||
