Effects of prostaglandin E_1(PGE_1) induced hypotensive anesthesia on intraoperative blood loss, blood transfusion volume, and renal and liver function were studied on 26 patients (34-75 years old) during orthopedic lower extremities procedures. The hypotension was induced by continuous drip infusion of PGE_1 untill systolic blood pressure drops to 90-100 mmHg under ethrane/N_2O anesthesia. During knee joint replacement procedure, blood loss and blood transfusion were significantly reduced in PGE_1 induced hypotensive patients (PGE_1 group; n= 6) compared with normotensive patients (control group; n=6). No significant change in postoperative deta indicating renal function was noted in PGE_1 group (n=12) compared with preoprative deta and that of control group (n=14). Also postoperative deta indicating liver function were within normal limit in PGE_1 group, except LDH. These results suggest that PGE_1 can be used safely in older patients to control blood pressure in order to reduce intraoperative blood loss and blood transfusion. |
